Transitioning to the home can be a very complex and challenging process for an individual and their family. March of Dimes Canada's After Stroke program is here to support everyone involved in the process.

Services include:

  • Hospital Visitation programs – Trained peers who have had a stroke and know what you are going through come to the hospital to talk and share their knowledge and experience about stroke recovery.

  • Home Adjustments Tips – Physical disabilities resulting from a stroke can make daily activities at home a challenge. View some tips to help make the transition back to your home a little easier. 

  • Assistive Devices Program – The program provides financial aid to help adults with physical disabilities after a stroke buy, repair and maintain a variety of mobile and assistive devices, including wheelchairs, scooters, walkers, aids for daily living and more.

  • Home and Vehicle Modification Program – Financial aid for eligible individuals to provide home and vehicle modifications. The program enables people to continue to live accessible, independent lives in their homes, avoid job loss, and participate within their communities.
